The individual health card (TSI) is the document that identifies and allows access to the centers and services of the assigned public health system.
Personal data for the management of the individual health card is part of an automated CatSalut file, called the CatSalut Central Population Register (RCA).
The main objective of this register is the recognition of the status of accredited person as well as the level of coverage associated with the citizens who appear in it.
The details of the accredited person are printed on the front of the card:
Personal identification code (CIP). It is the set of rules (expressed with numbers and / or letters) that, individually and unambiguously, allows to identify each accredited person of CatSalut.
Name and surnames of the holder.
Level of coverage. It is the level of health benefits that CatSalut assigns to each person.
The back of the card incorporates a magnetic stripe with encoded information from this printed data. Therefore, it is important to preserve it correctly (without bending it, or scratching it, or putting it in contact with magnets or on appliances connected to the mains).
The identification with the health card guarantees the confidentiality of the data of the accredited person.

Also, in most cases, it is necessary to register as holders or beneficiaries of the National Social Security Institute ( INSS ) or the corresponding mutual society ( MUFACE MUGEJU or ISFAS ) in the case of Spaniards or foreigners registered as to self-employed workers, pensioners, recipients of periodic Social Security benefits or similar situations and their beneficiaries (minors, spouses, newborns).
If you are not in any of these situations, you must process the application for affiliation with the INSS as Holder or Beneficiary and attach it electronically with your DNI / NIE . There is also the option to do so in person at the INSS by requesting an appointment by calling 901 10 65 70.
To this end, the sending of the TSI application implies that you authorize the Catalan Health Service to consult the data to other administrations or public entities in order to resolve your application, in accordance with the Article 6 of Organic Law 3/2018, of 5 December, on the protection of personal data and guarantee of digital rights, and Article 53.d of Law 39/2015, of 1 October, on administrative procedure common of public administrations.

You must fill in the application form and you can authorize the Catalan Health Service to consult your information online with other administrations, or bring the following documents:
- Identifying document (DNI, NIE, passport).
- Certificate of registration with the date of issue less than three months. Some municipalities have online consultation , but if your city council does not offer this service, you must bring the original certificate.
